What are social determinants of health?
Social determinants of health (SDOH) are the conditions in which people live, work, and spend time, which can influence up to 80% of health outcomes for patients. Information about SDOH can be useful in giving providers a view into other factors that influence their patients' health – such as housing, nutrition, transportation, and social support.
